Intake Process

Rooted Connections Counseling

PLEASE NOTE: We currently have approximately a 2 month waitlist for therapy services, particularly for appointments after 3:00 or weekends. We apologize for this inconvenience and extra stress, and look forward to being able to work with you and your family soon.

Reminder: Our practice is only in network with Blue Cross/Blue Shield PPO, and individual plans may differ. Please contact your insurance provider to verify that we are in network before starting services.

Contact Us

There are two ways you can connect with us: you are welcome to leave a message in our intake voicemail (773-417-6342 ext 1), or through this website. Either way, once you contact us, you will receive a reply from an intake therapist within 48 hours, or the next business day.  The intake therapist will schedule an intake screening call, which typically takes about 15 minutes. During this call, the intake therapist will ask you questions about your concerns, to help understand your needs and who may be the best fit to help. Your intake therapist will then follow up with you to connect you with your assigned therapist.

Please fill out the form below if you are interested in services. We look forward to speaking with you!

Please note:  Our waitlist is currently over 2 months long, particularly for appointments after 3:00 or weekends.  We are happy to speak with you about your concerns in the meantime through an intake screening, and will add you to the waitlist. We are sorry that we don’t have more availability at the moment, and look forward to being able to work with you and your family soon.

You will receive an email confirmation of this form submission. If you do not receive this confirmation email, please check your spam folder.

PLEASE NOTE: Submitting this form will not connect you with a mental health professional right away. We will do our best to reply to you as soon as possible, but it may take up to 48 hours, possibly longer if you are submitting over a holiday weekend. If you are experiencing a mental health emergency and cannot wait due to safety concerns, please contact dial 911, or 988 for the mental health emergency line, or visit your closest emergency room.
